Does the key hole work, or is it just cosmetic?

The key hole is integrated into the handle assembly. If your MDX uses the door handle to unlock the door, this pull will function the same way — test-fit first to confirm locking mechanism alignment with your door latch.

Can I use this on either front door, or only the left?

This is the left-side pull. Right-side pulls have a mirrored key hole placement. Order the correct side for your installation point — mixing sides will cause alignment issues.
